Each patient is rendered as a FHIR R4 bundle with real vocabulary codes; the ingestion adapter normalizes it into the exact signals the engine reads, with provenance. The bundle is a synthetic system-of-record (no live Epic wire) — but every engine value traces back to a coded resource.
